Abstract

The Brazilian epidemiological scenario points out that chronic non-communicable diseases are increasingly common and primary health care is the level of care considered as the foundation for the health system. The aim of this study was to analyze the actions developed in primary care for people with chronic diseases in the state of Rio Grande do Norte, Brazil. This is a descriptive, cross-sectional study of a quantitative nature, produced from secondary data of public access from the 3rd cycle of the National Program for Improving Access and Quality of Primary Care (2015-2017). The data were analyzed using the Statistical Package for the Social Sciences (SPSS) software and the frequencies were determined. It was found that only 36.3% of the primary care teams perform the fundus examination in people with diabetes and that the action most performed by the teams after identifying users with obesity is referral to the specialized service (89.0%). The main strategy carried out by the expanded family health and primary care center for people with chronic diseases is motivation and guidance for self-care (98.2%). The analyzed actions were shown to be satisfactory, especially an improvement in results when compared in the literature with previous cycles.
